Clinic accepts referrals from the Provincial Health Registry (managed in Burnaby by the Burnaby Division of Family Practice), community partners, and self-referrals.<\/p>\n<p><strong>Patients with valid MSP<\/strong>, the Burnaby Division of Family Practice pre-screens registrants from the Provincial Health Registry and assigns to the MOSAIC Community Clinic. Therefore, individuals are encouraged to ensure they are registered with the \u2060<span style=\"color: #008080;\"><a style=\"color: #008080;\" title=\"https:\/\/hcr.healthlinkbc.ca\/s\" href=\"https:\/\/hcr.healthlinkbc.ca\/s\">Provincial Health Registry<\/a><\/span>.<\/p>\n<p>Registration takes less than five minutes and requires a Personal Health Number (PHN), home address, email, and phone number. We would be happy to assist anyone who is having difficulty accessing or completing the Health Registry application. At a minimum, we require the individual&#8217;s name and contact information, and we will follow up to gather any missing details. Referral forms can be emailed, faxed, or dropped off at the clinic.
